Home and Community Care Home Modification Services
Home and Community Care Home Modification Services are funded through the joint Commonwealth/State Home and Community Care Program administered by Queensland Health. The Department of Housing manages and administers 10 Home Modification Services under a Memorandum of Understanding with Queensland Health.
The aim of the program is to assist older people and people with a disability to remain at home.
Services provided
Home and Community Care Home Modification Services may provide information, assessment, project management and/or financial assistance for larger home modifications, such as ramps and bathroom adaptations.
The range of services provided may include:
- initial home assessment;
- occupational therapy assessment;
- arranging the preparation of building plans and specifications;
- cost estimation;
- project coordination of building modification;
- post modification assessment; and
- referral and information.
Target groups
As the services are funded by the Home and Community Care Program, eligibility is determined by the Home and Community Care Program National Guidelines. The service is for older and frail persons and younger persons with moderate, severe or profound disabilities, who need support with home modifications to prevent premature or inappropriate long-term residential care in a hostel, nursing home or similar institution, and the carers of those persons.
