Freedom of Information application form (35Kb)
A Freedom of Information application must be in writing and provide such information concerning the
document as is reasonably necessary to enable an officer to identify the
document.
Freedom of Information amendment of personal information application form (30Kb)
If you have had access to a document from an agency or Minister containing
information relating to your 'personal affairs', or the 'personal affairs' of a deceased person to whom you are next of kin, you can apply to an agency or Minister for amendment or correction of any part of the information if it is inaccurate, incomplete, out of date or misleading.

If you are not satisfied with any decision about your application for Freedom of Information, you have a right to ask for a review.
